BHES is currently using a patient portal to securely send and receive all patient forms. If you need to complete any BHES patient forms, please
contact our administrative staff at 815-609-1544, inform them of what you
are wanting or needing to accomplish, and then our administrative staff
will send you the necessary form(s) to your patient portal for completion.

If you have not already set up your patient portal account, please inform
our administrative staff of this and they will assist you in getting it set up. Once it is set up, the requested patient forms will be sent to your patient
portal account for completion and submission.

All BHES patient forms are now completed through the patient portal and
securely sent to us when you have completed them.
